Appointment Information

Appointments are based on referral. Referrals must come from a family doctor or specialist.

New Patients

  • Nephrology

    • Arrive 15 minutes before appointments, bring all medications, and any pertinent medical information. Fee billed for No Show or Cancellation with less than 5 business days notice.

  • Psychiatry

    • Virtual APPT - Login to OTN 5 minutes before your appointment. Ensure all your documents are dropped off prior to your appointment. All caregivers are to be present for initial consults. The cancellation fee is $250.

Reminders

  • Please go to your primary care provider if you have concerns when our office is closed for psychiatry as there is a joint collaborative care agreement with Dr. Marcia Benjamin

  • Voicemails will be checked throughout the week

  • Physicians can contact Psychiatry, Dr. Marcia Benjamin, through OTN to ask questions through the consult launch link

After Hours

  • In case of emergency dial 9-1-1 or go to your nearest emergency room

  • Non-urgent - leave a message on voice mail will be answered in one to two business days

 

Current Patients

  • Nephrology

    • No smoking 2 hours before appointments and bring all medications. Ensure your lab work was done 1 week ahead of time. Fee billed for Cancellations/No Show with less than 5 business days notice.

  • Psychiatry

    • Come 20 minutes before your appointment. If there are new or crisis issues please try to inform us 1 business day in advance so we can plan accordingly. Fee billed for Cancellations/No Show with less than 5 business days notice.

 

Referral Process

  • Referrals have to be sent in by your primary care physician, nurse practitioner, or other medical physician. The referring doctor will collaborate with us in following our consultation recommendations. They will be triaged based on medical severity and openings on the clinical roster. For psychiatry, if the referring physician wants interim recommendations, please have them send an e-consult requests through OTN Hub link above.

  • Wait-List new patients, please check with your referring doctor as recommendations were by Dr. M. Benjamin.

  • We will contact you to book an appointment. Both parents, teacher, and child are required to complete questionnaires prior. Once questionnaires are turned in appointment will be booked.

  • Psychiatry follow-up is determined by Dr. M. Benjamin.

  • Referrals can be faxed to: (705)-728-8476. Or sent through OTN.

 

OTN Consultations:

  • OTN (Ontario Treatment Network) Consultation referral can be sent to Dr. Marcia Benjamin through the referring physician’s ONEID via econsult.

  • Response is 2-3 business days
